Legacy Tiered Certification Application

Legacy Requirements

CCHW II

Certified CHWs who have been active in the field for the equivalent of at least 4 full-time years and for at least 1 year in CHW leadership, may apply to the SCCHWCC to be legacy certified at Level II. The documents that should be submitted include:

  • Documentation of CHW Certification. Certification may have been acquired in another state, in which case the Subcommittee will review the core competencies included in the certification training program and determine if the certification requirements align with those of South Carolina and can be accepted.
  • Evidence of 4 full-time years (or the equivalent in hours) of work or volunteer experience, such as job descriptions, volunteer service descriptions and/or resume/CVs. Letters from a supervisor at a work or volunteer experience can be submitted, with a detailed description of the hours served and scope of work included. If the organization does not exist anymore, evidence of closure and a description of the scope, hours worked, and dates can be submitted.
  • Evidence of at least 1 year of CHW leadership experience, which may include as a CHW ambassador, SCCHWA board member, active involvement in SCCHWA workgroup, active involvement in CHW regional or national committee or initiative, CHW Preceptor, CHW supervisor, CHW program leadership or program development at an organizational level, or an equivalent.
  • A professional recommendation from a current or previous supervisor, or partner the CHW has worked with as a CHW, attesting to the CHW’s ability to perform at an advanced level.
  • Any other documentation requested by the SCCHWCC
  • A one-time processing fee of $25

CCHW III

Certified CHWs who have been active in the field for the equivalent of at least 8 full-time years and for at least 2 years in CHW leadership, may apply to the SCCHWCC to be certified at Level III

  • Documentation of CHW Certification. Certification may have been acquired in another state, in which case the Subcommittee will review the core competencies included in the certification training program and determine if the certification requirements align with those of South Carolina and can be accepted.
  • Evidence of 8 full time years (or the equivalent in hours) of CHW work or volunteer experience, such as job descriptions, volunteer service descriptions and/or resume/CVs. Letters from a supervisor at a work or volunteer experience can be submitted, with a detailed description of the hours served and scope of work included. If organization does not exist anymore, evidence of closure and a description of the scope, hours worked and dates can be submitted.
