You’re invited to join our Cardiovascular, Kidney & Metabolic Disease (CKMD) Advanced Training, presented by Constellation Quality Health in partnership with the South Carolina Community Health Worker Association.
This 16-hour, four-day hybrid training is CEU-eligible and designed to strengthen your skills in cardiovascular health, diabetes, kidney care, medication literacy, closed-loop referrals, self-measured blood pressure monitoring (SMBP), and client support.
Priority is given to Community Health Workers (CHWs) serving rural and under-resourced communities.
