Join us for the official launch of envision’s newest tool;  the Sustainability from the Inside Out Community Health Worker (CHW) Organizational Self-Assessment Toolkit. This tool is a practical, CHW-centered resource designed to help public health departments and others assess organizational readiness to support and sustain thriving CHW programs. Grounded in the CHW Common Indicators, this toolkit guides organizations through a process of identifying organizational readiness across four domains:

  • CHW Wellbeing

  • CHW Sustainability

  • CHW Program Integration

  • CHW Policy Environment

Participants will explore the toolkit and learn more about leading an assessment that honors CHW expertise, protects occupational integrity, and develops actionable pathways towards long-term CHW program sustainability.

Who Should Attend?

  • Community Health Workers (CHWs)

  • CHW supervisors, managers, and program directors

  • State, Tribal, Local, and Territorial (STLT) health department leaders and program coordinators employing or developing CHW programs

  • Community-based organization leaders employing or developing CHW programs
